Appointments
Our patients are seen on an appointment basis. Please remember to call in advance so that time with the doctor may be reserved especially for you. The office telephone number is 352-385-9156. Every effort is made to have the doctor see you on time, and we ask that you extend the same courtesy by arriving at the scheduled hour. Often, emergencies arise, and we will keep you informed of any delays. Please let the receptionist know if the delay causes a problem for you and she will be happy to reschedule your appointment at a more convenient time. Your cooperation is always appreciated.
Financial
Every effort is made to keep down the cost of your medical care. Payment is expected at the time services are rendered. We accept cash, checks, MasterCard and VISA. We participate with the Medicare program and most insurance companies. Check with our receptionist if you have any questions.
Insurances
We will file insurance claims for those patients who have insurance coverage. Therefore, on your initial visit we will ask that you provide insurance information and a copy of your driver’s license or government identification.
Please remember that insurance is considered a method of reimbursing the patient for fees paid to the doctor and is not a substitute for payment. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, co-insurances, or any other balance not paid by your insurance company.
When questions on insurance matters arise, please feel free to discuss them with the receptionist. She will be happy to help you receive maximum benefits; however, the agreement by the insurance company to pay for your medical care is a contract between you and your insurance company.
Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
